Alan Harper, played by Jon Cryer, is feeling down about his appearance and lack of success with women, so Charlie Harper, played by Charlie Sheen, decides to take him to his favorite cosmetic surgeon. While there, Charlie decides to get some work done too, but a mix-up with the medical records leaves Alan with Charlie's sexy new chin and Charlie with Alan's less-than-desirable nose.
As they adjust to their new looks, Charlie finds himself being hit on by women left and right, while Alan struggles to adjust to his newfound attractiveness. Meanwhile, Jake, played by Angus T. Jones, is going through some changes of his own as he prepares to enter high school and starts hanging out with a new group of friends.
As the episode progresses, the brothers get into some hilarious hijinks as they try to navigate their new appearances. Charlie takes advantage of his newfound looks, going on a string of dates with beautiful women and even getting offered a modeling job, while Alan struggles to even talk to women without getting tongue-tied.
