K's blood has kept the hotel's heart going for 15 years. Manami finds the room where K is kept. Akari has been keeping K informed about all that has gone on in the hotel. She has also told her that Manami has amnesia. K bites Manami to awaken her power. In the meantime, 2 cops stumble into the hotel. They've come to the hotel to investigate a series of murders. Cordy is their prime suspect. The hotel residents are astounded to learn that the rest of the world had not ended. In response they try to convince themselves that staying in the hotel will ensure their survival. They decide to feed the ancient Dracula blood to Akari so she can be the hotel's new heart. They'd do anything to keep the peace and comfort of the hotel. Just when Akari is about to drink the ancient blood, K storms in with a sword.

One step outside Tokyo Vampire Hotel is a radioactive inferno that goes forever. That's what everyone believes. But Cordy has casually been going there after he finishes work in the evenings. Tokyo is still a thriving big city and it isn't rubble after all. The world has not ended. Cordy the chef picks up women so he can drink their blood. Akari, soon to be 14, is desperate to know about the outside world. Soon she figures out that there is in fact a world outside. She asks Cordy to take her on one of his trips outside. As a birthday surprise, Cordy invites Akari outside. She breathes the air outside the hotel for the first time... Then she wakes up in the hotel. Was it a dream? But Cordy is gone, leaving only 1 week of food for them to survive on. The mortals will soon starve to death and the vampires will lose their blood supply. Meanwhile Manami and K wait for their power to awaken.

After Princess died, K became the hotel's heart and circulates blood. A new addition to the hotel is Akari (Nana Mori), a mortal girl who was born in the hotel. She'll soon turn 14. The hotel is her whole world. She's never been outside. Both the mortals and vampires adore her. And for the moment none of them is aware of the hotel's secret.

An escape attempt by the mortals fails. They question whether or not the world has ended. Yamada, now the King, opens the door to show them what's outside. Tokyo is an inferno as far as the eye can see. The mortals are convinced and become submissive. Yamada orders them to find partners to have sex with. Those who don't find a partner in the given time will die. With that threat hanging over their heads, the mortals randomly hook up and make love under surveillance. The Corvins want them to procreate so they'll have an endless supply of blood to feed off. That's the only way they can survive. In the meantime Manami is captured and bitten by Elizabeth. This awakens Manami's mind-blowing power. K on the other hand is pushed into a crack on the wall into an abyss. She finds herself with banished mortals who live in torment. And K learns the secret behind Tokyo Vampire Hotel.

The Draculas go to the surface to confront the Corvins while K and Manami go back to Tokyo Vampire Hotel. When K tells Noa to stay behind, Noa confesses to love her. Meanwhile at the Tokyo Vampire Hotel, the Queen dies and Yamada takes over her throne. The mortals unite and fight the hotel's vampire employees. K returns to the hotel and shoots incendiary arrows at the vampires' combustible shadows and chops them up with a sword.

Together they help Manami escape and take her to the Draculas deep underground. Manami learns her destiny, that she was born on the day of the Grand Cross in 1999. Two other babies were born on the same day but neither of them could endure the Dracula's potent ancient blood. One got sick and died while the other lost her mind and killed herself. Only Manami survived to the age of 22. And K is a messenger sent to deliver her to the Draculas.

A Japanese exchange student named K and her Romanian friend Noa, visit an ancient castle in Transylvania. There's a myth that the castle was occupied by vampires who used it as a hotel to lure mortals. The two young women meet a young woman who takes them into a cave that's supposedly connected to the entire world. They end up at Hotel Requiem. The Dracula clan played a central role in vampire myths up until the 16th century. Then the Corvin clan, a new breed of vampires, imprisoned the Draculas deep underground. The Corvins have ruled above the ground since. Centuries later in 1999, the planets in the solar system aligned in a Grand Cross. On that day at 9:09, 9 seconds, three babies were born. According to the Book of Prophecy these babies would save the Draculas from doom. The children would reach their maximum potential on their 22nd birthdays in 2021. Manami (Ami Tomite), one of the children, will turn 22 in three hours. She celebrates her birthday with friends at a tavern but then an enigmatic assassin named Giga (Shoko Nakagawa) kills everyone except Manami. She escapes the killing spree but she is captured by a woman named K (Kaho) who tells Manami to obey her or else. Manami escapes K and is rescued by Yamada (Shinnosuke Mitsushima), who turns out to be a Corvin vampire.
