Run's Surprise Confession is the sixteenth episode of the first season of To Love Ru. In this episode, Run, a popular transfer student, is tasked with writing an article about the school for a magazine. However, she becomes very nervous and self-conscious, as she doesn't want to disappoint her editors. Meanwhile, Rito and Lala are trying to figure out why Run is so afraid of failing despite her popularity.

As they investigate, Rito and Lala stumble upon Run's secret: she suffers from a split personality disorder and is afraid that if she fails, her other personality, Ren, will take over and ruin her reputation. Rito and Lala decide to help Run by encouraging her to write the article and reminding her that they like her no matter what.

As Run begins to write, Ren takes over and confesses her love for Rito, much to his surprise. Lala, who had been trying to convince Rito to confess his own feelings for her, becomes jealous and sets out to sabotage Ren's advances. The three of them end up in a hilarious competition to win Rito's affections.

Meanwhile, the other characters have their own problems to deal with. Yami, the assassin sent to kill Lala, continues to cause trouble, but is slowly beginning to see the value of friendship. Mikan, Rito's younger sister, is dealing with her own schoolwork and crushes. And the eccentric scientist, Dr. Mikado, is up to his usual experiments and inventions, which never seem to go according to plan.

Overall, Run's Surprise Confession is a fun and lighthearted episode that showcases the show's comedic and romantic elements. It also highlights the importance of understanding and accepting others, especially when they have hidden struggles or secrets.
