The episode begins with Doc being woken up by his ex-girlfriend, Tamara, who tells him she needs his help. Tamara's brother, Nicky, has been released from prison and is desperate for money. He has stolen a car and is now holding a family hostage in their home. Tamara asks Doc to intervene and help her brother out.
Meanwhile, Sully and Ty respond to a call about a man trapped in an elevator. When they arrive, they discover that the man is actually someone Sully knows - his old partner from when he worked in narcotics. The man, Eddie, is in bad shape and begs Sully for help. Sully agrees to take him to the hospital, but Eddie warns him that the people he used to work for are still after him.
Back at the precinct, Carlos and Grace are dealing with a stubborn horse that has gotten loose in the city. The horse has caused a lot of damage and Carlos is determined to catch it. They eventually manage to corner the horse, but not before it causes even more damage.
As the day goes on, Nicky becomes more desperate and violent. He shoots a police officer and takes a woman hostage. The entire precinct is mobilized to deal with the situation. Meanwhile, Sully is struggling to keep Eddie safe from the drug dealers who are after him.
In the climactic scene, Nicky has holed up in an apartment building with his hostage. Ty, Sully and Doc go in to try and rescue her, but things quickly go wrong. There is a shootout, and several people are injured. In the end, it's up to Doc to talk Nicky down and convince him to surrender.
The aftermath of the incident leaves everyone shaken. Doc is left to wonder if he did the right thing in helping Tamara, while Sully is forced to confront his own past as a narcotics detective. The episode ends with a somber reminder of the dangers faced by those who serve and protect in a big city.
