Mulder and Scully arrive at a small Massachusetts town to investigate a series of bizarre deaths that have been occurring. Each victim appears to have died from a heart attack, but upon closer inspection, it's discovered that a swarm of tiny insect-like robots have been burrowing their way into the victims' brains, causing them to collapse from the inside out.
As Mulder and Scully begin their investigation, they realize that the town is in the midst of a mysterious epidemic. Not only are people dying from these strange heart attacks, but others are reporting strange sightings of metallic bugs and UFOs in the sky. Mulder is convinced that extraterrestrial forces are involved, while Scully remains skeptical and suggests that the phenomenon could be the result of a covert government operation.
As they dig deeper into the case, Mulder and Scully encounter a variety of eccentric locals - including a bug-obsessed entomologist and a paranoid radio talk show host who believes the government is using mind control techniques to manipulate the population. Meanwhile, Mulder begins to experience hallucinations that could be the result of the nano-machines infiltrating his own brain.
As the investigation leads the duo to a secret government facility, they discover that the true source of the epidemic is even more terrifying than they could have imagined. Now, Mulder and Scully must race against time to stop the spread of the nano-machines before it's too late. But with the entire town suddenly turning against them, their mission may prove to be more difficult than they ever imagined.
