In episode 5 of season 1 of The Indian Doctor, titled "Heatwave," the summer heat takes its toll on the residents of the Welsh village of Trefelin. The episode opens with a scene of an intense heatwave sweeping across the village, and the locals are struggling to cope. The lack of rain has resulted in a severe water shortage, and residents are rationing their usage to ensure the supply doesn't run out.

In the midst of this, Dr. Prem Sharma (played by Sanjeev Bhaskar) faces a medical emergency when a farmer falls ill. The farmer's condition gradually deteriorates, and Dr. Sharma suspects heat exhaustion. However, he quickly realizes that there may be more to the case when he discovers that the farmer had been using a banned pesticide on his crops. As Dr. Sharma races against time to save the farmer's life, he also has to deal with suspicion and hostility from the locals who blame him for the worsening situation.

Meanwhile, a wealthy businessman named Richard Sharpe (played by Mark Heap) arrives in Trefelin to open a bottled water factory. He promises to alleviate the water shortage by providing jobs and free water to the villagers. However, not everyone is convinced of his good intentions. Dr. Sharma and his wife, Kamini (played by Ayesha Dharker), are wary of Sharpe's motives and suspect that he is more interested in making a profit than helping the villagers.

As tensions rise, Dr. Sharma finds himself caught in the middle of a conflict between the villagers and Sharpe. He tries to navigate the situation as best he can, but it seems that he can't please everyone. Meanwhile, Kamini, who is a teacher at the local school, starts to experience her own troubles when her pupils start to exhibit bizarre behavior in the heat. She suspects that they are suffering from dehydration and decides to take matters into her own hands.

As the episode unfolds, we see how the heatwave affects different characters in different ways. Some are driven to desperate measures to get water, while others struggle with their own personal issues. Throughout it all, Dr. Sharma remains a steady presence, trying to help where he can while dealing with his own frustrations and limitations.

Overall, "Heatwave" is a tense and compelling episode that highlights the challenges faced by a small community in the face of a natural disaster. It also highlights the ways in which people come together in times of crisis, despite their differences and personal struggles.
