Sam and Dean arrive in an Iowa town to investigate a series of murders that are connected to a legend of a ghostly figure called the Hook Man. According to local lore, a minister's daughter was dating a man her father disapproved of. The couple made out in the back of a car that was parked in the woods. As they kissed, the man's hook hand scraped the roof of the car. When they arrived back at her house, the girl's father discovered the hook on the door handle and realized that the man was the notorious Hook Man, a criminal with a hook hand. The minister then killed the man with the hook. Ever since then, the Hook Man has been rumored to haunt Lovers' Lane and kill those who engage in sexual activity.
Sam and Dean interview one of the victims' roommate, who tells them that their friend, Lori, saw the Hook Man. The brothers then head to their Lori's dorm to check up on her, but she's not there. Noticing that the room's windows and doors are locked, the brothers grow increasingly suspicious and decide to break in. They find Lori in the bathroom, cowering in fear, while the Hook Man attacks Dean with his hook. Sam manages to burn the ghostly figure with a lighter and immediately tells Dean that the Hook Man must be a spirit that they can salt and burn.
The brothers then head to the local library to research the legend. They discover that the minister's daughter, Rebecca, was institutionalized after her father killed the Hook Man. Furthermore, they learn that the minister himself also died in a fire at the church. The Hook Man's corpse is then exhumed and burned to ashes by the brothers. However, the killings continue as they soon realize that the Hook Man is actually the ghost of Rebecca. She's seeking revenge on those who break the commandment, "Thou shall not commit adultery."
Sam and Dean figure out that Rebecca's ghost can't cross on to hallowed ground and is therefore confined to a small area. They devise a plan to lure her into a church with Dean as bait. They then salt and burn Rebecca's remains and destroy the hook.
As the episode comes to an end, Sam and Dean drive off into the night, commenting on their latest successful hunt. Dean praises his brother, saying that he's like Batman, while Sam muses on whether they'll truly be able to continue their work of "hunting things" forever.
