When the Enterprise crew receives a distress signal from an unknown planet, they're met with a puzzling image: a humanoid face, distorted and contorted beyond recognition. As they try to decipher the message, Troi begins to experience some unusual sensations, feeling as though the distorted face is reaching out to her specifically. Meanwhile, Data is conducting a field study on a neighboring planet when he too comes across the same strange image.
As the two different stories begin to converge, the crew realizes that they're dealing with an extremely rare and dangerous phenomenon - a creature that can manipulate reality and bend it to its will. As the creature begins to exert its power over the Enterprise crew, they must find a way to stop it before it takes over their ship and possibly the entire Federation.
