Clark, Lois, and Jason are at the local museum attending a benefit function. Clark spots an old photograph and recognizes one of the women in it- it's his mother. He talks to the curator, Heidi, and she reveals that the woman in the picture was once her best friend, Patricia Swann. Patricia was a brilliant scientist who was involved in the study of anti-aging, but she disappeared under mysterious circumstances many years ago.
Clark and Lois investigate Patricia's disappearance and discover that she has been holed up in her mansion since the 1950s, apparently using her own anti-aging serum to stay young and alive for all these years. However, the serum is slowly killing her, and she has been searching for an antidote.
The episode has several subplots running in parallel. One of them is Lionel Luthor, who is trying to track down the Traveler. He confronts Martha and tells her that he knows about Clark's secret. Martha is forced to leave the farm and go into hiding, leaving Clark feeling helpless and angry. He confides in Patricia, who gives him some advice on how to take control of his life.
Meanwhile, Lana is still trying to come to terms with the death of her parents, and she is struggling with her own mortality. She pays a visit to Patricia and learns about the anti-aging serum, which tempts her to try it herself. However, she ultimately decides that she doesn't want to live forever and chooses to embrace the uncertainties of life.
In the climax of the episode, Clark discovers that Lionel has been using meteorite fragments to track him down. He confronts Lionel and they have a tense standoff. Clark realizes that he doesn't have to run anymore and that he has the power to face his enemies head-on.
The episode ends with Patricia dying peacefully, having finally found the antidote to her serum. Clark takes comfort in the fact that she has left behind a legacy that will help people live longer and healthier lives. He also realizes that he doesn't have to be afraid of growing old, because he will always have his family and friends by his side.
