Jerry Seinfeld and Elaine Benes attend an operation where Jerry's latest girlfriend is undergoing surgery. Meanwhile, Kramer has a new friend named Lomez who works at a candy factory and offers him and Jerry a tour of the facility. During the tour, while eating Junior Mints, Jerry accidentally drops one into the open surgical cavity of the patient's torso without anyone noticing.
After the surgery, Jerry and Elaine are caught in the hospital waiting room by the patient's boyfriend, Roy, who is a well-known competitive runner. Roy is angry and confronts Jerry about his lack of attentiveness towards his girlfriend's wellbeing, leading to a heated exchange between the two. Meanwhile, the patient's condition begins to worsen and the doctors discover the mint inside her body, but are unable to remove it.
Jerry becomes fixated on the incident, believing that the Junior Mint may be the key to saving the woman's life. He insists on visiting her in the hospital and keeping an eye on her condition, frequently sneaking into the room to check on her. This leads to Elaine teasing him about being overly invested in a woman he had only been dating a short time, causing Jerry to question his feelings and the purpose of his relationships.
In the end, the events surrounding the Junior Mint incident come to a head when Jerry and Roy have a high-stakes showdown during a charity run.
