J.D. and Turk are roommates at the hospital and spend a lot of time together. However, Turk struggles to keep his relationship with Carla a secret from J.D. and is afraid he'll accidentally reveal the truth. Meanwhile, J.D. has his own problems as he tries to keep his big mouth under control.
At the beginning of the episode, J.D. is holding a staff meeting and blurts out that Dr. Kelso's son is gay, despite promises of confidentiality. Dr. Kelso is furious and threatens to fire J.D. if he can't prove his worth as a doctor.
Meanwhile, Turk has his own problems as he is struggling to keep his relationship with Carla a secret from J.D. They try to come up with excuses for why Turk is spending so much time away from their apartment, but it's clear that J.D. is starting to get suspicious.
J.D. enlists the help of Elliot to improve his relationship with Dr. Kelso. Elliot, who has a bit of a crush on J.D., is eager to help but quickly realizes that J.D. has ulterior motives. J.D. tries to impress Dr. Kelso by suggesting a new treatment plan for a patient, but it backfires when the patient dies.
Meanwhile, Turk is struggling to keep his secret from J.D. and ends up confiding in Carla. She's upset that Turk doesn't seem to trust her, but Turk explains that he's just scared of losing his friendship with J.D. Carla suggests they tell J.D. the truth, but Turk is still hesitant.
As the episode progresses, J.D. has a few more mishaps that put him on Dr. Kelso's bad side. He accidentally reveals a patient's diagnosis to their family and gets into an argument with Dr. Cox.
Turk finally tries to tell J.D. about his relationship with Carla, but J.D. interrupts him to reveal that he's been accepted into a prestigious medical program. Turk is happy for J.D., but realizes that he still hasn't been able to tell him the truth.
