Hank is approached by a billionaire businessman who believes his father's sudden stomach issues may be more serious than he is willing to let on. Hank soon discovers the patient is a bio-extremist who purposely became contaminated in order to bring attention to an important cause, and the case takes a turn for the worse when we learns that the toxins in his body are not only harming him, but also those around him.
Meanwhile, Evan and Paige celebrate their first anniversary and come up with an elaborate plan to surprise each other, only to have things go awry. And Divya embraces her new role as mother to her baby boy, but struggles with the decision of returning to work and leaving her son in the care of others. Hank must call on his medical expertise, as well as his personal relationships, to help all three of his loved ones through their respective challenges.
Throughout the episode, the importance of communication and trust is highlighted in relationships and in medical treatment, as Hank navigates through the complexities of his patients and family members' lives. The storyline also touches on the themes of sacrifice and the greater good, as the bio-extremist's decisions and actions ultimately affect not only himself, but also those around him.
