Hank and Divya are presented with a new challenge when a group of wine collectors, who have gathered together to sample some of the world's best wines, begin to fall ill with strange symptoms that are not consistent with any known poison. Hank and Divya race to investigate the source of the illness while also navigating interpersonal conflicts and scandals within the group of collectors.
Meanwhile, Evan takes on a new role as the interim head of HankMed while his brother Hank focuses on the mystery illness. Evan quickly finds himself in over his head when he must navigate both the administrative and medical sides of the business, all while trying to maintain the family's reputation.
As Hank and Divya dig deeper into the illness, they discover that the wine the collectors were tasting was not the rare vintage they thought it was, and instead may have been tainted with counterfeit ingredients. With time running out and the illness spreading, Hank must use all of his medical knowledge to find a cure before the situation turns deadly.
Throughout the episode, the characters struggle with their own personal and professional challenges. Hank must come to terms with his growing feelings for Jill, while also navigating the complicated dynamic between himself and his former business partner, Boris. Divya tries to balance her responsibilities to HankMed with her personal life, as she navigates a difficult long-distance relationship with her fiancé. Evan struggles to prove himself as a capable leader, all while dealing with the fallout of a business decision that goes awry.
