Tommy, Lou, and Needles go on a road trip to help break a fallen Jim out of a mental institution. On their journey, they discuss their own personal struggles with mental health and the challenges they face as firefighters. Meanwhile, back in New York City, the rest of the crew try to pick up the pieces after the tragic events in the previous episodes. Sheila copes with her guilt and grief over the loss of her baby, while Franco struggles with his anger issues and his relationship with Natalie.
