Quincy, M.E. season 4 episode 4 titled "Death by Good Intentions" follows the medical examiner, Dr. Quincy, as he investigates a suspicious death at a rehabilitation center. The episode begins with Quincy receiving a call from his friend, Dr. Auerbach, who works at the rehab center. Dr. Auerbach informs Quincy that one of his patients, a former actress named Phyllis Todd, has died unexpectedly and he suspects foul play.
Quincy arrives at the rehab center and begins his investigation. He learns that Phyllis was recovering from alcohol addiction and had been making good progress. She had even been chosen to star in a play that the rehab center was putting on for its patients. However, on the night of the performance, Phyllis collapsed and died.
Quincy examines Phyllis's body and discovers that she had a high level of a medication called Imipramine in her system. Imipramine is an antidepressant that is sometimes prescribed to patients with alcohol addiction to help with anxiety and depression. However, Quincy suspects that Phyllis was given too much of the medication, which caused her death.
Quincy speaks with the staff at the rehab center and discovers that there is a major problem with the distribution of medication. The rehabilitation center is understaffed and overworked, which has led to mistakes being made in the distribution of medication. Quincy suspects that one of the staff members gave Phyllis too much Imipramine by mistake, causing her death.
Quincy's investigation leads him to a young counselor named Jim, who had been giving Phyllis her medication. Jim is devastated by the accusation, but Quincy believes that he has evidence to prove that Jim was responsible for Phyllis's death. However, when Quincy presents his evidence to the police, they are not convinced. They believe that Quincy's evidence is circumstantial and that Jim is innocent.
Quincy is frustrated by the police's lack of action and decides to take matters into his own hands. He continues his investigation and discovers a new piece of evidence that he believes will prove Jim's guilt. Quincy confronts Jim with the evidence and convinces him to confess to accidentally giving Phyllis too much medication.
The episode ends with Jim being arrested and Quincy feeling a sense of closure. Despite the tragedy of Phyllis's death, Quincy is glad that justice has been served and that the staff at the rehabilitation center will be held accountable for their mistakes.
