Set in sunny Los Angeles, Private Practice follows the lives of a group of doctors who run a private medical practice called Oceanside Wellness Center. The show is a spinoff of Grey's Anatomy, and its second season welcomed a few new characters to the cast.

One of the more compelling medical cases in this episode involves a young woman named Sarah (Amanda Seyfried), who is suffering from severe headaches and hallucinations. The Oceanside doctors perform tests and discover that Sarah has a brain tumor that could potentially kill her if left untreated. Sarah is resistant to surgery, but the doctors work to convince her to undergo the operation.
Throughout the episode, we see tension building between some of the doctors at Oceanside Wellness Center. Sam and Naomi's ex-husband William (James Morrison) have a disagreement over how to treat a patient, and Addison and Wyatt get into an argument about their relationship. Addison begins to realize that she and Wyatt may not be as compatible as she initially thought.
The episode ends with a few dramatic developments that will likely carry over into future episodes. Naomi makes a decision about her love life, and Addison's relationship with Wyatt takes a surprising turn.
