In Private Practice season 2 episode 16, titled "Ex-Life," the characters are dealing with the consequences of their past decisions. Addison Montgomery (Kate Walsh) is faced with her ex-husband, Derek Shepherd (Patrick Dempsey), who arrives at Oceanside Wellness Group seeking treatment for his hand after a car accident. This leads to tension between Addison, Derek, and her current boyfriend, neurosurgeon Sam Bennett (Taye Diggs).
Meanwhile, Violet Turner (Amy Brenneman) is struggling with her feelings for her ex-boyfriend, Cooper Freedman (Paul Adelstein), while he is preoccupied with caring for his ailing mother. Cooper is worried about his mother's health and is having a difficult time dealing with the reality that she is dying. Violet wants to be there for Cooper but is afraid of getting hurt again.
In addition, Charlotte King (KaDee Strickland) confronts her past when her abusive ex-husband, Keith (Michael Patrick Thornton), comes to the hospital with injuries sustained from a motorbike accident. Charlotte grapples with whether or not to help him, leading to intense emotions as she reflects on their violent relationship.
Amidst all of these personal struggles, the doctors at Oceanside Wellness Group must also tend to their patients. One of their patients is a man in need of a heart transplant who is waiting for a donor to become available. Another patient seeks the help of Addison and Naomi Bennett (Audra McDonald) after learning she is pregnant with twin boys and one of the babies has Down Syndrome.
Throughout "Ex-Life," the characters must navigate their complicated personal lives while also providing the best medical care for their patients. The episode features themes of love, loss, redemption, and forgiveness.
