As a young girl, Tsunade made a pact with her two younger brothers to always be by each other's side and protect each other no matter what. But after a tragic mission that went wrong, her brothers were killed and Tsunade has never been able to forgive herself for their deaths. Flash forward to the present, and Tsunade is faced with a difficult dilemma: Orochimaru has returned to the village and is threatening to destroy it unless Tsunade meets his demands. In order to save the village, Tsunade must relive the trauma of her past and make an impossible choice: to use her medical skills to revive Orochimaru's former partner, Hiruzen, or let him die.
The episode opens with Tsunade locked in a heated battle with Orochimaru, who is determined to get what he wants from her at any cost. But just as he's about to deliver the final blow, Jiraiya and Naruto arrive to save the day. Tsunade is grateful for their help, but she knows that their victory is only temporary.
After regrouping, Tsunade fills Jiraiya and Naruto in on her past and the mission that cost her brothers their lives. Finally, Tsunade makes her decision. She agrees to use her medical skills to revive Hiruzen, but only on the condition that Orochimaru leaves the village and never returns. Orochimaru agrees to her terms, and Tsunade sets to work. The process is difficult and emotionally draining, but Tsunade is determined to see it through.
