The group planning the birthday party is led by Lee Ik-jun, a lovable and quirky doctor who always carries a sense of humor wherever he goes. Seok-hyeon, a quiet and reserved obstetrician who is best friends with Ik-jun, is the one celebrating his birthday.

Ik-jun's plan to surprise Seok-hyeon with a cake while on duty is thwarted multiple times, and the group eventually ends up celebrating at a traditional Korean restaurant, where they are joined by their fellow friends and associates. One of Ik-jun's patients, who is experiencing complications with her pregnancy, has unexpectedly arrived at the restaurant. This episode also explores the dynamic between Jeong-won, a pediatrician, and his friend Gyeo-ul, a general surgeon. Gyeo-ul tries her best to get Jeong-won to explain his decision to leave the priesthood. However, Jeong-won keeps evading her – it is apparent to the viewers that he has a fervent passion for being a pediatrician.

The episode also features a case of a patient who has been referred to Ilsan Hospital by another facility. The group of friends takes on the difficult case of a young girl, whose mother is an overseas immigrant worker. The girl's condition is critical due to complications from a botched surgery performed at the previous facility, which caused her to go into septic shock.

This case presents logistical and financial issues – the girl's mother works at a factory and, as an immigrant worker, doesn't possess medical insurance. The show touches on the complex nature of healthcare in Korea by showing the various ways this situation affects the patient's treatment, recovery, and financial burden.

Apart from these cases and situations, the episode showcases the group's friendship – their emotional support for each other, their ongoing discussions about their personal and professional lives, and their unwavering dedication to their patients.

Additionally, this episode gives an insight into the characters' love lives. While Ik-jun's and Seok-hyeon's love lives remain a secret, two other doctors' romantic relationships take center stage. One of their new interns, Jang Gyeo-wool, is shown interacting with Lee Ik-sun, Jeong-won's sister, in a potential love match. The other doctor, Chu Min-ha, a lovable and driven doctor who is a recent addition to the team, finally gets the attention of a patient with whom she has previously hit it off.
