Danny and Chin Ho head over to the beach to talk to the identified homeless man. Here, they find his belongings hidden inside one of the destroyed trash cans. The team soon discovers that the homeless man, who was involved in a car accident, has suffered from a traumatic brain injury, leading to increased aggression and a troubled relationship with the police ever since. The team realizes that the man was not the kidnapper, but rather a witness to the events that took place on the day the car owner went missing.

The team realizes that the kidnapper is someone who has a grudge against the medical community, as evidenced by the specific type of trash cans that were targeted. The team's investigation leads them to a disgruntled ex-employee of the hospital where the victim was to receive his transplant. The man had access to the victim's medical records and schedule and had planned to take revenge by withholding the victim's donor organ.

The case reaches a swift conclusion as the team locates the victim and the kidnapper who has him hostage, and the victim successfully receives his transplant. The team manages to arrest the kidnapper before he can execute his plan of revenge.
