When Dr. Brown learns that his estranged brother Ephram is coming to Everwood to play at a nearby ski resort, he is both excited and apprehensive. Ephram has always been the black sheep of the Brown family, and their relationship has been strained since their father's death. As Ephram arrives in town, he seems to be trying to make amends for past mistakes. He starts by apologizing to Amy for his insensitive behavior towards her in the past, and she is surprised by his sincerity.

However, not all of Ephram's efforts to reconnect with his brother are successful. When he tries to talk to Andy about their father's death, Andy becomes defensive and shuts him down. This leaves Ephram feeling frustrated and hurt, and he seeks solace in the company of his new bandmates, whom he met at the ski resort. Encouraged by his new friends, Ephram decides to move back to New York and focus on his music career. This decision leaves Andy feeling guilty and regretful, but he is unable to express his true feelings to his brother.

Meanwhile, Bright is trying to come to terms with the fact that his girlfriend Hannah will be leaving for college soon. He wants to make the most of their remaining time together, but Hannah is focused on her studies and is less interested in spending time with him. This creates tension between them, and Bright feels like he is losing her. Eventually, Hannah realizes the extent of Bright's feelings for her and makes a romantic gesture to show that she cares.

In addition to these personal dramas, the episode also deals with several medical cases. Dr. Abbott is treating a young boy with a rare brain disorder, and his parents are struggling to come to terms with their son's condition. Dr. Abbott forms a close bond with the family as he tries to help them navigate the medical system. Elsewhere, Dr. Brown is called to a local mine where a worker has been injured. The situation is complicated by the fact that the man's boss is also his brother, and they have a strained relationship. Dr. Brown must navigate this tension to provide the best care possible for his patient.

Overall, "My Brother's Keeper" is a deeply emotional episode that explores themes of family, loyalty, and regret. The relationship between Andy and Ephram is at the heart of the episode, and the actors do a wonderful job of portraying their complex feelings towards each other. The subplot involving Bright and Hannah adds some lightness to the episode, and the medical cases provide an interesting counterpoint to the personal drama. This episode is a great example of the kind of complex storytelling and nuanced character development that made Everwood such a beloved show.
