Sherlock Holmes, a brilliant but troubled detective, has just been released from rehab and moves from London to New York City. He meets his new sober companion, Dr. Joan Watson, an ex-surgeon, and an unlikely partnership begins to form between the two.

In this pilot episode, Captain Gregson of the NYPD hires Sherlock to help investigate a bombing in Central Park that left three people dead. Meanwhile, Sherlock is still struggling with addiction and has to attend a support group meeting. He shares his story and mentions a time when he almost died from an overdose, implying that he had attempted suicide. This reveals the depths of his emotional struggles and his past traumas as a drug addict.

The episode climaxes with the discovery of the person responsible for the bombing, revealing a connection to both the victim and the Holmes family. It also ends with a heartwarming scene where Sherlock decides to continue working with Watson as his sober companion despite her completing her original contract with him.
