The premise of Dr. Steve-O was for Steve-O to perform crazy stunts and pranks on unsuspecting people, while also promoting safety and raising awareness about various health issues.

The show consisted of Steve-O and Trishelle traveling to different locations, such as a skate park or a basketball court, where Steve-O would perform a dangerous stunt or prank on an unsuspecting participant. The stunts ranged from jumping over a row of people on roller skates to flying off a ramp on a bicycle while wearing a tutu, to performing a backflip onto a pile of broken glass. Trishelle played the role of the safety advisor and would help Steve-O set up the stunt and make sure safety precautions were taken.
