Tomoya and Nagisa visit the high school's library and encounter a girl named Fuko Ibuki who is carving wooden starfish. Fuko introduces herself as Nagisa's younger sister, though Nagisa denies having any siblings. Tomoya learns that Fuko is actually a year younger than Nagisa and has been carving starfish to be used as wedding favors for her sister's wedding. However, no one seems to know who Nagisa is getting married to.
Later, Tomoya and Nagisa visit Fuko at the hospital where she is carving the starfish. They learn that Fuko's body is physically present, but due to an accident, she is unable to attend school and attend her older sister's wedding. Instead, Fuko had hoped to hand out the wooden starfish at her sister's wedding in her place.
Tomoya takes on the task of helping Fuko complete her wooden starfish carving project. He enlists the help of his friends Kyou and Ryou to speed up the process. As they continue carving, Fuko's health gradually deteriorates, and Tomoya begins to wonder if he is the only one who can see her.
As the day of Nagisa's sister's wedding approaches, Tomoya begins to question whether they should attend. He worries that attending the wedding may cause Fuko more pain and agony due to the fact that she needs to hand out the starfish that she worked so hard to carve.
Tomoya finally decides that they should attend the wedding and makes plans for the wooden starfish to be passed out in Fuko's place. However, as the wedding day arrives, Tomoya is forced to come to terms with the reality that he cannot see Fuko anymore, and that she may not wake up again.
The episode ends with Fuko's wooden starfish serving as a heart-warming tribute to the girl who worked so hard to create them. Tomoya and Nagisa watch as the starfish are handed out as wedding favors, and Fuko's presence is felt by the family and guests who take them home.
