The second patient featured in the episode is Derek, a transgender man who previously underwent top surgery to remove his breasts. However, he is unhappy with the results and feels that his chest feels too flat and masculine. He meets with Dr. Dubrow and Dr. Nassif, hoping to get a revision surgery to create a more natural-looking chest that aligns with his gender identity.

The doctors determine that Derek's previous surgery left him with significant scarring and that his nipples were not properly repositioned. To fix these issues, they plan to perform a revision surgery that involves repositioning his nipples and using tissue from his stomach to create a more natural-looking chest.

Throughout the episode, viewers are given an inside look into the surgeries and consultations, as well as the emotional journeys of both patients. Marcela expresses her fears and insecurities about going under the knife again, while Derek opens up about his struggles with body dysmorphia and the importance of feeling comfortable in his own skin.
