Danny Reagan and Maria Baez investigate the case of a young woman who was found dead in her apartment after an apparent drug overdose. The victim was identified as Ava Coulter, a nursing student with a bright future ahead of her.
As they delve deeper into Ava's life, they discover that she was struggling with chronic pain and had been prescribed a powerful opioid painkiller by a doctor who had a history of overprescribing. As they dig even further, they discover a network of doctors and pharmaceutical companies who are profiting from the opioid epidemic.
Meanwhile, Frank Reagan deals with a similar case when a young man is found dead from an overdose, and his mother is desperate for answers. Frank is determined to find out who enabled her son's addiction and hold them accountable.
At the same time, Erin Reagan juggles her responsibilities as a prosecutor as well as a single mother to her daughter, Nicky. She is forced to make a difficult decision when she is offered a promotion that would require her to relocate to Albany, which would mean uprooting Nicky's life as well.
Jamie Reagan finds himself in a dangerous situation when he tries to help a woman who is being harassed by a group of men. He quickly realizes that they are members of a local gang and must use his training as a police officer to diffuse the situation before it gets out of hand.
As the Reagans work to solve their respective cases, they must also confront the opioid epidemic that is tearing apart their city and their country. They grapple with the complex issues surrounding addiction, pain management, and the role of pharmaceutical companies in the crisis.
Ultimately, the Reagans are reminded of the importance of family, solidarity, and doing the right thing in the face of difficult circumstances.
