There's a woman who's convinced she's psychic and has started making decisions based on her "visions," a man who's afraid of getting his hair cut, and a woman who's been struck by lightning multiple times and is now afraid of thunderstorms.
Becker manages to help each of these patients in his own gruff way, but his irritation with Bob continues to grow throughout the episode. He even starts to hallucinate about Bob playing the drums whenever he hears a similar sound.
As the episode progresses, it becomes clear that Bob's drumming is masking some deeper problems. Finally, Becker decides to give up his anger and try to reach out to his old friend. In an unexpected twist, he discovers that Bob is suffering from depression and substance abuse.
The episode ends on a bittersweet note as Becker tries to be there for his friend, even as he struggles with his own annoyance and anger. Throughout the episode, the themes of friendship, mental health, and the importance of reaching out for help are all explored in Becker's usual sardonic style.
