Donna tricks Cleveland into going to see the doctor for a checkup, despite Cleveland’s fear of the doctor. Once they get inside, Cleveland finds himself enjoying Dr. Fist (voiced by Bryan Cranston from “Breaking Bad”).

At school, Rallo isn’t happy with the lunch that Junior packed for him. Junior explains that he tried a new approach to lunch; a deconstructed peanut butter and jelly sandwich. He tastes a piece and loves it. He decides to open up a food truck with Junior and sell their sandwiches. A rival food truck shows up and steals business from them.

While relaxing at home, Cleveland gets a call from Dr. Fist, saying his test results were in and that he wants to do more testing. He tells Cleveland to get there as fast as possible.

At the bar later that week, Cleveland complains to the men that Dr. Fist has had him come in every day for more tests, and he’s swabbed everything, but he won’t tell Cleveland what’s wrong. The guys think he’s dying. Cleveland vows to complete his bucket list before it’s too late. First off, eating ice cream for breakfast.

At the bar, he crosses the ice cream off his bucket list, which, incidentally, was the only thing on his bucket list.

Donna tries to reassure Cleveland that he’s not going to die. As she tells him, Dr. Fist shows up at the door. He tells Cleveland that he has bad news for him; that he’s been lying. He just wanted to hang out with him, and the tests were pointless. Dr. Fist asks Cleveland to forgive him. Donna asks why Dr. Fist didn’t just ask him to hang out, and he said that doctors can’t just ask patients to hang out. Cleveland asks him for a beer. Dr. Fist tells him to come by his place that week.

Rallo and Junior confront the competing food truck, telling them to stay out of their turf. They laugh and say there’s no way. Rallo vows to find a way to get them to leave.

The guys show up at Dr. Fist’s house, and end up having a good time. Cleveland realizes that Dr. Fist is rich and tells him that he wants to be friends now. He asks Dr. Fist what happened to all his old friends, and Dr. Fist reveals that he dropped his cell phone in a toilet and managed to lose all of his contacts, never hearing from them again. They celebrate by playing life-sized Operation.

Rallo visits Kendra and asks if she has any roaches he can use to get the other food truck shut down. She supplies his a bag of them for a small fee.

The guys and Dr. Fist hang out in the park, and Dr. Fist reveals embarrassing medical facts about various people who walk past them. Cleveland and crew load into his car, because he has a surprise for Dr. Fist. He tells him he felt bad that he lost all his friends, so he put his picture up on the internet and managed to track down a group of his old friends. Dr. Fist panics because they are actually the mob and they want to kill him.

Dr. Fist explains his story. He says he was a private doctor and got roped into aiding the mafia exclusively. He was later approached by the FBI and asked to help take down the mob boss, and they agreed to put him into witness relocation.

The rival food truck driver confronts Junior, saying that her truck was shut down by the health department, and asks if he had anything to do with it. Rallo gloats, and Junior gets angry, because he didn’t want to play dirty. He tells Rallo he has to fix things, or else.

Outside the bar, Cleveland flags down a car that just happens to contain the mobsters in it. They lead him inside the bar at gunpoint. Cleveland reveals that he’s helping the Mob and shoots Dr. Fist in the chest, where he falls to the floor. The mobsters are surprised and verify that he’s dead before leaving, and telling Cleveland he’s one tough man.

After they’re gone, Cleveland reveals that Dr. Fist and he planned it, and it wasn’t a fatal wound. He tries to have Dr. Fist sit up, but he’s not moving.

Rallo brings the health inspector back the truck and tries to convince him that the cockroaches were actually the latest trend in cooking. To prove it, Junior freezes one in liquid nitrogen and makes Rallo eat it. The health inspector is convinced and allows the truck to reopen.

In the bar, Cleveland struggles to get Dr. Fist to come to. He realizes that they need to get the bullet out of his chest for him to survive. Cleveland realizes that they can save him by playing Operation on him like before. Three hours later, he’s fine, and says that he wants to stay in town and be one of the guys. They celebrate.
