The Orphanage is a supernatural thriller film about a woman who returns to the same orphanage that she was adopted from many years ago. Her dream is to get the abandoned orphanage up and running for a home for disabled children. Laura(Belen Rueda), along with her husband, Carlos(Fernando Cayo) and son Simon(Roger Princep), move into the building and begin to renovate. Simon befriends a boy that wears a sack on his head. The boy, Tomas teaches a game to Simon where he discovers that Laura is not his real mother, he is HIV positive, and he is adopted.
Simon is angry upon the discovery and disappears during a party at the orphanage. Thinking that it is the social worker, Benigna(Montserrat Carrat) who had previously visited has taken him, Laura and her husband search frantically for the boy. They get the police involved with no leads.
Six months pass and Laura happens to see the social worker with a baby stroller. She is accidentally killed. Laura finds in the stroller a doll with a sack over its head. It is revealed that the social worker worked at the orphanage when she was younger and her son was the boy with the sack over his head.
Tomas was drowned when the other children stole his sack mask and hid it in the caves. The tide came in and killed him. The social worker took revenge and killed the children that was responsible for stealing his mask. The children's spirits haunt the orphanage. Laura begins to communicate with the children by playing a game with them. With the help of the spirits, she discovers a closet that lead to a basement.
She finds a blanket and Simon's remains. She is absolutely heartbroken when she realizes that she accidentally caused his death by blocking the door. Heartbroken she overdoses on medication and kills herself to be with her son.
