Beginning where the last film left off, Rose and Sharon are trapped in Silent Hill, which is in another dimension. Rose manages to contact her husband through a mirror and use an amulet to send their daughter back to him. Sharon has no memory of what has happened and her father tells her that her mother died in an accident. He then changes their names and moves to avoid the cult that is searching for her. Named Heather and Harry while in hiding, they stay on the move to avoid detection. Meanwhile Heather keeps having nightmares about the Silent Hill amusement park and being set on fire by Alessa.

On her way to school, Heather is questioned by Douglas, a private investigator hired by the cult to find her. She manages to avoid giving him any information, and then meets Vincent, who is a student at her new school. The school turns into an alternate reality monstrosity, then changes back. After school she sees Douglas again and calls her father, who is then taken by something unseen.

Heather goes to the local mall to wait for her father when reality changes again and she then sees a monster kill Douglas. The world changes back to normal, she sees the corpse and runs away. Once she gets home she sees a message on the wall telling her to go to Silent Hill. She takes the amulet and with Vincent leaves for Silent Hill as the police show up. Once there, Vincent tells her he is the cult leader's son, and that he was supposed to bring her to them but was not going to due to her being the good half of Alessa. He tells her to go to his grandfather who is locked in an asylum, and then he is dragged off by a monster as Heather is knocked out.

Vincent is labelled a traitor and sent to the asylum to be cured as Heather is getting there. She meets Leonard, Vincent's grandfather, who tells her the amulet reveals what things really are. He then jams the half she has into his chest to join it with the other half in=side him. Then he changes into a monster whom she defeats and retrieves the amulet.

Heather is taken by the asylum patients, but is saved by a monster with a pyramid head. She then finds Vincent strapped down and surrounded by horrible looking nurses. She unties him and they go to the amusement park where Heather's father is being held. She runs into Alessa, and they fight for control, with Heather absorbing the evil half of herself.

The cult leader and Vincent arrive when Heather finds her father, and the leader tells her she was meant to destroy Alessa and birth the cults new God. Then the leader touches the amulet and turns into the same monster that killed Douglas. Heather is attacked but saved by the pyramid head monster again who takes off the leader's head. The ashes stop falling in Silent Hill, and Heather and Vincent prepare to leave. Her father chooses to stay behind and try to find his wife. As the pair leave, police cars show up and the ash begins to fall again.

Silent Hill: Revelation 3D is a 2012 thriller with a runtime of 1 hour and 34 minutes. It has received poor reviews from critics and viewers, who have given it an IMDb score of 4.9 and a MetaScore of 16.
