In the small town of Harmony, a tragedy occurred on Valentine's Day ten years ago. A miner named Harry Warden went on a killing spree after he was trapped inside the mine and had to resort to cannibalism to survive. The townspeople believed Harry to be dead but the murders stop once he is presumed dead. Ten years later, the town is preparing for Valentine's Day festivities and is also reopening the mine after being closed for a decade. Tom Hanniger (Jensen Ackles) returns to his hometown after 10 years of being away. He was the one who caused the accident which led to Harry's killing spree and the deaths of several other miners. His return to town is met with mixed emotions from the town folk who still haven't forgotten his past. Tom is trying to make amends for his past mistakes, but everyone in town seems to remember the tragic events of that Valentine's Day and the role he played in them.

Shortly after his arrival, people in the town begin to be killed one by one by a figure wearing a miner's suit, proving that Harry Warden might still be alive. The killings coincide with Tom's return, and he becomes the prime suspect. Tom, aided by his former girlfriend, Sarah (Jaime King), tries to clear his name and uncover the truth behind the murders. The mayor, played by Kerr Smith, is determined to carry on with the festivities despite the murders happening in the town.
